The initial investigation at the scene of the crash of the 'Owl' helicopter near the Philadelphia Highway in Rafah indicates that the squadron did not identify the landing pad due to dust rising from the ground. >>>We are now recruiting to help Einav get married and establish a faithful home in Israel. לפי הדיווח הערב (רביעי) ב'כאן חדשות', הטייסת, שנפצעה קשה מאוד בהתרסקות, דיווחה על הבעיה בשעה 00:32 הלילה. עוד עולה מהתחקיר, כי הטייסת החליטה להמריא מהמקום ולבצע נחיתה חדשה. בשלב זה, הלהבים ברוטור של זנב המסוק פגעו בסוללת עפר. המסוק נטה בחדות שמאלה, הלהבים העליונים פגעו בקרקע, והמסוק התרסק מגובה מטרים בודדים ונחבט בקרקע בעוצמה. מאחר שלוחמי 669 לא היו יישובים וחגורים בכיסאות, כחלק מהתרגולת לפני פינוי פצועים משטח לחימה - הנפגעים נחבטו בתוך תא הנוסעים.

גורמים בחיל האוויר ששוחחו עם 'כאן חדשות' העריכו כי האסון נגרם מטעות אנוש וכי הסיכויים שהייתה תקלה טכנית במסוק נמוכים. בחיל האוויר אספו אחרי האסון את חלקי המסוק, שיסייעו בהמשך חקירת התאונה. &nbsp
